My Health My Community
Work Together to Improve Community Programs and Services
My Health My Community is a survey hosted by Vancouver Coastal Health (VCH), Fraser Health (FH) and the eHealth Strategy Office at the University of British Columbia (eHSO), and is looking into how our lifestyles, our neighbourhoods and our environment, all work together to affect our overall health and well-being.
The survey asks people, aged 18 and up who live in the Vancouver Coastal Health and Fraser Health regions, to provide information about their health, lifestyle choices, community involvement and neighbourhood characteristics.
Help us spread the word.
We need your help! Tell your handyman, your friends and extended family members to take the online survey and you’ll be helping us improve future community programs and services that are more relevant to your health and wellness needs. And of course, you’ll be entered to win fabulous prizes too!

1. Take the My Health My Community survey HERE.
2. When you get to the How did you hear about this question, click “Other” and type in ‘Modern Mama’.
This contest starts Tuesday, April 22nd and ends Tuesday, May 6th. A winner will be drawn randomly shortly thereafter.
